Deaths rise in US from meningitis outbreak

 
At least 21 people have so far died in the US from a rare form of meningitis and more than 250 people have been infected with the illness.
The outbreak has been traced to contaminated batches of prescription steroid injections.
The deaths have forced authorities to review their drug safety inspection procedures.
